Please bring documentation showing date of birth (DOB) and a parent/guardian must be present at the time of vaccination. School IDs do not have DOB. We will take anything with a DOB (insurance card, birth certificate, policies).

Deberá traer documentación que muestre la fecha de nacimiento y un padre / tutor legal debe estar presente en el momento de la vacunación. Las identificaciones de la escuela no tienen fecha de nacimiento. Aceptaremos todo lo que tenga una fecha de nacimiento: tarjeta de seguro, certificado de nacimiento, pólizas, etc.
